Basic

Add custom fields to an existing form or create a brand new form to gather additional information from your clients and prospects. Our basic form includes creating one to ten custom fields such as "sign up to receive my newsletter" or adding a comment box to receive questions.

Advanced

Create an advanced custom form with eleven or more fields to gather client or prospect information that is specific to your needs. For example, you may want to create a custom form that informs you about your client's interests or concerns, a change in their status, or areas by which you could provide more service. With the advanced form, you can also add your logo to give it a personal touch.
